The Suicide Registry Instruction Manual is a reference document to be used by clinical departments managing cases of suicides and other deaths for defining cases, collecting data and checking the data being collected. The Instruction Manual is intended to be used in conjunction with the Case Report Form (CRF – specifically the “Suicide Report Form” for the NSRM) to ensure uniformity in reporting. This document should be kept at hand when doing data entry or checking. A PDF version of this document is available in the training CD distributed to all Source Data Producers. This manual will also form the basis of training for suicide reporting for the NSRM. |
